Systems for Wrap-Around Support
Goal: Create a better experience for youth and their families and provide the right resources to those who need them most
Theory: If each organization knows what resources are being offered to a youth and their family, then they can better coordinate the provision of those services and work through trusted relationships to support the family more effectively.
Strategies
Unified System
Take advantage of Microsoft’s donation to MBSKCO in order to provide a suite of technologies to community-based organizations (CBOs) that support their needs, encouraging widespread adoption and common infrastructure
Success Measurement
Replicate / integrate the processes that Montbello Organizing Committee (MOC) has established for its mental health programs. Recruit other interested orgs to help us generalize those methods to apply to the wide range of community programs offered.
Case Management
Replicate / integrate the system that Life-Line Colorado uses to define the case management functionality that will both support program operations and enable referrals, partnering CaseMgr and building on the robust efforts of the Latino Coalition
Legal
Put dedicated work into creating a module that can collect release of information forms from families and help them understand what they are signing and the implications
Iterative Expansion
Recruit additional community-based organizations to use the system and continue to extend functionality to ensure it can generically support all who would wish to use it, building on the investment of above orgs
